You will find dates for the H1N1 (swine) influenza booster clinics for all children under the age of ten. For information about the vaccine, please refer to the vaccine information sheets you received previously, during the first round of vaccinations. •  Fill out the consent form and return ASAP to your school nurse.
      If you do not fill out this permission  form completely, your child will not be given the vaccine.
