PROCEDURES FOR REQUESTING INFORMATION FROM STUDENT RECORDS

In compliance with Chapter 125, Section 19, regarding parental/guardian access rights to student records and confidentiality rights of parents and students, Windham Middle School wishes to inform parents that student records shall be available to you for inspection and copying through the following procedures:

 

1. Parents/guardians are to make inspection or copying requests through the guidance or main office.

2. A time, mutually agreed upon by the school and parent/guardian, will be determined.

3. Parent/guardian will review the records in the school in the company of school personnel. Records are not to be removed from the school.

4. Upon reasonable request by parent/guardian, copies of school records will be made available.

5. Parent/guardian may schedule a meeting with administration to discuss student records that parent/guardian believes are incorrect.

6. Student records will be kept in fireproof filing cabinets located in the guidance office.

7. Student academic records will contain the following types of information:

Testing, report cards, progress reports, PET minutes, IEPs, medical, Attendance, behavioral, and personally identifiable information.

8. All procedures and regulations pertaining to student records are in accordance with district-wide policy regulation 5218 and state law Chapter 125, Section 19.
