Kindergarten registration for Fall 2013 will open on Friday, February 1st. You can register for a screening time by going to the Windham Primary School website starting on February 1st. The dates for screening are the following:
Thursday, April 11, 2013 & Friday, April 12, 2013
There will be no kindergarten sessions for current kindergartners on these days so that the kindergarten staff may operate the state mandated screening. We appreciate families’ support with the screening process.
Dear Parent/Guardian:
The kindergarten staff looks forward to meeting you and your child at your Screening Appointment. This will include: a review of immunizations, birth certificate, proof of residency, registration forms (provided online), an interview with the school nurse, vision and hearing screening, DIAL-4 (a developmental assessment) as well as a parent orientation. Your child will be asked to participate in a variety of tasks designed to acquaint the faculty with his or her strengths and learning style.
The state law requires that:
1. You present a Certified Copy of the Birth Certificate issued by the town where your child was born or where you lived at the time of your child’s birth. A hospital certificate does not meet the state requirements.
2. Children entering kindergarten must have Written Proof of Immunizations for DPT (5 doses), IPV/OPV (4 doses), MMR (2 doses) and Varicella (1 dose) from your health care provider. If you have no records, please call your doctor and ask for a Certificate of Immunization. Any waiver of immunization requirements must be approved by specific documentation and discussed with the school nurse. [Maine School Immunization Law, Title 20-A, requires that a child must provide a Certificate of Immunization within 90 days of enrolling.]
It is also the policy of the RSU14 Windham Raymond School District that:
3. Children must have a physical examination prior to entering school. A form is provided for you to take to your child’s appointment. It is found online in Part 3: Additional Forms.
4. You must provide proof of residency at the screening appointment. (Examples of proof would be: current utility bill, current mortgage or rental receipt, or current property tax bill, with your physical address printed on it. A driver’s license is not acceptable.)
